Here is a list of forms that are available for you to download.  If you do not have Acrobat Reader, please click the link below as you will need to have Acrobat Reader in order to read these files.

 

Step 1:  Application Packet

All perspective students must fill out an Application Form, submit two Recommendation Forms, and a Request for Transcript Form from previous school, if applicable.  The Admission Committee will review all Application Packets to select the student roster.

Step 2:  Registration Forms

Parents will be informed by the Admission Committee when their student is selected to attend Big Horn Christian.  Once placed on the official student roster, parents must submit all Registration Forms. 

All NEW students enrolling for the 2009-2010 school year must fill out the following forms:

Conduct Contract

Emergency Contact and Medical Information Form

Over-the-Counter Medication Form

Student Medical Record Form

(For all new students)

Health/Immunization Form (Pick up from local doctor, printout of their immunization record)

Internet Acceptable Use Policy Form

Parent Release Form for Media Recording
